On April 30, 2013, Home & Community Services (formerly UCP Home & Community Services) ended Service Coordination services. We wish to thank our consumers and employees for the years of support. It has been our pleasure providing this service to children and adults with disabilities throughout Washington, Greene, and Fayette counties.

If you are considering in-home personal care services, please contact Pathways of SW PA's In-Home Services Department at 1-866-629-1638. Pathways provides in-home personal care to assist children and adults with disabilities, as well as senior citizens, live as independently as possible in the comfort of their own home and their familiar surroundings. To learn more, go to Pathway's website.
